Tournament
Information sheet for general information about the tournament.
If your team is interested in participating, proceed to step 2. |

Complete the
Tournament Entry Form. (Click on the link to access the online
form.) |
| 3. |
Complete the
Tournament Roster Form. (Information provided will be verified
against official league records.) |
| 4. |
If any non-roster players
were added in step 3, complete the Roster
Change Form. |
| 5. |
Complete the
Tournament Waiver Form. Please make sure that you
obtain the necessary signatures for ALL participants. No one
will be allowed to play without the proper signatures on file. |
| 6. |
For 12th grade teams
only: Complete the Conduct Form. Please make sure that you obtain
the necessary signatures for ALL participants and for the designated
organizational officers.

Submit the
completed Tournament Waiver Form, Conduct Form (12th grade teams only),
and a check for the
Entry Fee ($600 thru March 31st; $650
after March 31st) to:
